This packet is designed to be used by a child's relative who is a Brother, Sister, Grandparent, Aunt, Uncle or Cousin. This packet may not be used by a natural mother. A natural Father may use this packet only when the natural mother's whereabouts are unknown.

The filing fee for this packet is $400.00. You must serve the natural parents with a Summons and there is a $10.00 fee per Summons, to have each Summons issued by the Clerk.

  Final Disposition Form
This form may be used to ask the court to enter a support order if you or your spouse are separated, and your spouse has the ability to contribute to you and your minor child(ren), but has failed to do so. You can only use this form if a dissolution of marriage has not been filed and based upon the time-sharing schedule, you are entitled to support.
