What is an arraignment?
An arraignment is a court date at which the defendant is advised of charges against him or her. A plea of not guilty or guilty is entered. The court also addresses the defendant's ability to obtain an attorney.
How can I obtain a Delayed Birth Certificate?

1. You must present a certified statement from the State Registrar of the alleged state of birth, stating that, based on the facts submitted by the petitioner, a birth certificate for the petitioner is not on file. |

2. You must furnish three documents with your date of birth. All three documents must show the same date of birth. |

Acceptable Forms of Evidence are: |

· Military Service Record
· Census Record
· School Record
· Application for Insurance Policy
· Affidavit from the attending Physician or Midwife
· Application for Marriage License
· Birth Certificate of Petitioner's Child
· Social Security Application
· Voter's Registration Record |

3. If the evidence is sufficient, the court shall issue a Delayed Birth Certificate on a form furnished by the department. Documentation submitted by the petitioner in support of the petition shall be recorded on the Delayed Birth Certificate. |

4. The original and court copies of the Delayed Birth Certificate issued by the court shall be distributed as follows: |

1. One copy shall be filed in the Circuit Court as a permanent record. |
|
|
2. If the birth occurred in Florida, one copy shall be delivered to the petitioner and the original shall be mailed to the department by the Clerk of the Court within 10 days after the Delayed Birth Certificate is issued by the court. |

|
3. If the birth occurred outside the state of Florida, the original certificate plus one copy shall be delivered to the petitioner by the court. |
|
|
4. A Delayed Birth Certificate issued by a court pursuant to this section and registered with the department may not be amended except by court order. |
|
|
5. The fee for issuing a Delayed Birth Certificate is $5.00. |

The cost for a criminal record search is $1.00 for each year to be searched, $1.00 for each copy, and $2.00 for certification. |

|
$77.00 to the Clerk of Circuit Court and $250.00 to the 4th DCA. Funds must be cashier's check or Money Order. |
