Forms you will receive regarding your jury duty:
Summons
A legal notice delivered to citizens stating that they are required to report to the courthouse for jury duty.
Juror Excusal/Disqualification Form (which is attached to the summons)
A form that is to be dated and signed with the juror number and phone number requesting to be excused from jury duty if criteria is met by the reasons listed on the form.
Juror Questionnaire
Perspective jurors can print this form and fill it out, then bring the completed sheet with them on the day of their service to submit to Jury Assembly Staff. This form provides basic background information relating to spouses, employment, law enforcement officers in the family, prior litigation experience, and potential disabilities that might preclude service on a jury.
Payment Affidavit
A sworn affidavit of juror's employment and whether or not they are receiving wages while on jury duty.
Exit Questionnaire
A form given to citizens in which they evaluate their experience after they have completed their term of service.
Certificate Of Jury Service
A certification of day(s) of the juror's service (this form is at juror's request and is usually for the employer's benefit). It must be signed and sealed by the Clerk of Court Deputy Jury Clerk in order to be valid.
Medical Request for Excusal
A form to be filled out by physician or nurse practitioner that prevents the juror from serving due to medical reasons.
