If you have a course provider that is not on our list, we have a Premarital Course Provider Affidavit (Click Here for form). You or the course provider can pick up the affidavit at this department or print a copy from this site. It must be filled out, notarized and then returned to our office in either Fort Pierce or Port St. Lucie.
